This campaign from the NHS aims to:
- encourage and support more conversations between people who provide health and social care and the individuals, families and carers who receive that care
- shift the focus from ‘what is wrong with you’ to ‘what matters to you’
The campaign is encouraging people to have a ‘What matters to you?’ conversation and then tell their story by answering three simple questions – Who did you talk to? How did it feel? What happened next? Stories can be found here where three videos from Brazil indicate the global nature of the day!
This approach to health and social care ties in with SIS’s Social Prescribing Service which supports health and social care practitioners to offer a more holistic service. It links patients to support within the community that can meet the social, emotional or practical needs that are effecting their well-being and health. Volunteer Linguists and Bilingual Community Navigators meet with patients to discuss their needs and create an action plan together which includes facilitated referrals to other specialist organisations, services and social groups.
