Service User K came to us for help with accessing emergency accommodation as he was facing imminent eviction and was at risk of homelessness. This was causing him high levels of stress and anxiety.
Our Community Navigator provided extensive support, liaising with the Homelessness Prevention Officer at the council and helping to fill in forms to ensure timely access to emergency accommodation.
She then also helped to complete the following forms to help Service User K to achieve his ultimate goal of securing permanent Sheltered Accommodation:
1. Assisted bidding Form
2. Homemove Housing Register Form
3. Mobility Form
4. Self Assessment Medical Form
5. Sheltered Housing Form
The Community Navigator also helped Service User K to collate all the necessary supporting medical, financial and identity documents for the application.

Service User K was moved into emergency accommodation.
Since then Service User K has been in touch again to say that he has now moved into permanent Sheltered Accommodation – he is extremely pleased with this outcome.
